Add panel help to each too item

Each tool under the too menu item was created independantly and there is no consistency between them. For this ticket makes sure all panels include a help button in a reasonably similar place. No toolbars or menus are used (except in the two special tools below the separator bar — those need a bit of cleaning up as well, but at least help works now).

Also fix positioning and size for usability and consistency.
